General study(2023 KDDW) Reports associated with Autoimmune Gastritis

2021년부터 2023년까지 stomach neoplasm으로 내원 한 환자 중 APCA 양성인 경우는 Gastric cancer: 6.9%, Gastric adenoma: 6.5%, Neuroendocrine tumor: 25% 에서 관찰되고 있으며, 전형적인 endoscopic finding인 reverse atrophy가 확인된 환자는 APCA양성 환자 중 13.2% 였습니다. Stomach neoplasm 환자들 중에서 autoimmune gastritis에 의한 경우가 underestimated 되어 있는 것 같습니다.

건강검진 center에서 2012년부터 2023년까지 내시경 검사와 혈청검사를 받은 환자들 중 22명에서 APCA 양성인 autoimmune gastritis 환자를 확인하였습니다. Reverse atrophy는 59.1%에서 확인 되었고, gastric neoplastic lesions은 9.1%, neuroendocrine tumor 는 27.2% 였습니다. Autoimmune gastritis 환자들이 clinical field에서 underestimated 되고 있으며 보다 주의깊은 관찰이 필요하겠습니다. 진단 기준, 치료의 표준화, 검사 기간 등에 대한 consensus가 앞으로 필요하겠습니다.

Atuoimmune gastritis는 progressive disease로 reverse atrophy가 확인 된 환자들 중에서 약 7년 이상 과거 endoscopy 기록을 통해, endoscopic atrophic score 를 만들어 비교한 연구 입니다. (현재까지 endoscopic atrophy의 내용은 Kimura-Takemoto classification (H.pylori associated gastritis) 만 있는데, lesser curvature / greater curvature 의 atrophy 정도를 0~3점까지, fundus atrophy유무를 0, 1점으로 scoring 하여 과거 내시경 검사 사진을 토대로 합산하여 점수를 매겼으며, 이후 내시경 소견에 따른 atrophy scoring 변화를 비교한 연구 입니다)
